2018-2019 Mini-Grant Applications Available

We are pleased to announce our 2018-2019 mini-grant program.  Awards of up to $4,000 will be made to community partners in our region to support programs focused on either of two areas described below.

Area 1 – Health Professions Students

Goal:  Utilize hands-on experiences to improve health professions students’ skills and competencies necessary to improve health care quality and provide team-based care in rural and underserved settings with an emphasis on primary care.

Area 2 – Practicing Health Professionals

Goal:  Improve practicing health professionals’ skills and competencies necessary to improve health care quality and provide team-based care for rural and underserved settings and populations.

Learning opportunities for health professions students or practicing health professionals that involve one of more of these AHEC core topics are encouraged:

  • Interprofessional Education and Practice
  • Behavioral Health Integration
  • Addressing Social Determinants of Health
  • Developing and Practicing Cultural Competency
  • Practice Transformation (Quality Improvement, Population Health, PCMH, etc.)
